Methodology. This document counts three categories of state-attributable death: (1) Direct — deaths from military operations, assassinations, coups; (2) Sanctions — documented excess mortality from economic blockades and healthcare collapse; (3) Fallout — post-invasion state collapse, infrastructure destruction, and conflict seeded by state action (e.g. al-Qaeda enabled by the CIA Afghan programme; ISIS enabled by the 2003 Iraq power vacuum). Where figures are disputed, the most conservative peer-reviewed estimate is used and the dispute is noted. All totals are floors, not ceilings.

Israel figure corrected to ~261,000+ covering the full 76-year period (1948–2025), including Lancet-estimated indirect deaths in Gaza 2023–25. The full toll is still accumulating.
Iran figure corrected to ~60,000–78,000 covering Yemen (apportioned share only), Syria, Iraq militias, Hezbollah, and direct attacks. Yemen is a multi-party conflict — the Saudi coalition (US/UK-backed) carried out 25,000+ airstrikes and is responsible for the majority of direct killings; Iran's share reflects Houthi ground violence only.
Iraq sanctions figure uses the peer-reviewed floor (Garfield ~350,000); the higher UNICEF 500,000 figure remains contested by three subsequent independent surveys.
